Physical Therapist (PT) – Contract Assignment

Contract Duration: 13 weeks
Location: Lebanon, PA

Job Description

We are seeking an experienced and dedicated Physical Therapist (PT) for a contract opportunity within a skilled nursing facility setting. The PT will be responsible for evaluating patients, developing individualized treatment plans, and delivering high-quality rehabilitative care to support mobility, recovery, and overall functional improvement.

Key Responsibilities

  • Perform comprehensive evaluations of patients' physical abilities and functional limitations.

  • Develop and implement evidence-based treatment plans tailored to patient needs.

  • Provide therapeutic exercises, mobility training, and manual therapy interventions.

  • Document evaluations, progress notes, and treatment outcomes in accordance with professional standards.

  • Collaborate closely with interdisciplinary team members to coordinate patient care.

  • Assist with discharge planning and provide recommendations for continued therapy.

  • Educate patients and caregivers on safe mobility techniques and home exercise programs.

  • Adhere to all regulatory, ethical, and facility-specific guidelines.

Qualifications

  • Active state license as a Physical Therapist (PT).

  • Prior experience in a skilled nursing facility preferred.

  • Strong assessment, communication, and clinical decision-making skills.

  • Ability to work both independently and alongside a multidisciplinary team.

  • Commitment to delivering compassionate, patient-centered care.
